If I fill a 12 ounce glass with draft beer there is foam and I do not pour a full 12 ounces. What is the industry standard for under-pouring?

Get a bigger glass.

Is this a personal observation or is there a scientific study that you can refer me too? My trials with a 12 oz glass are similiar - 10 oz of brew with a 3/4 inch head.

Personal observation. A lot of restaurants tend to have small glasses, high prices, and limited experience with beer.
Not /all/ restaurants, of course...
