Great Lakes Brewery in Cleveland.
For Chicago, there's "Piece" (brewpub with guest beer), the Hop Leaf (beer bar), and Goose Island (brewpub; I prefer the Clybourne street location) as my top destinations in Chicago proper. There are bunches of other places in the greater Chicago area. If you can push past Detroit, I'd add suggest Bell's Eccentric Cafe in Kalamazoo.
Detroit Beer Company, downtown Detroit. 2-3 blocks from Comerica Park and Ford Field.

In addition, the Hop Leaf is the place to go if you're fond of Belgians, both on draught and in bottle. The food is also quite good.
Also, just a few blocks from Piece (great pizza there, btw) is another seminal Chicago beer bar, The Map Room.
One of my favorite places. Several other brewpubs in Kalamazoo, as well, but The Eccentric Cafe is THE place.
